Transforming Intergrated Care Boards and Sub-Groups

Integrated care is part of current health care reforms and focuses on more coordinated and integrated forms of care provision.

Integrated care is care and support built around the needs of the individual, their carers and family. A central aim of integrated care is to create cooperation and coordination between health, social care, public health and other local services and the third sector.

The Health and Wellbeing Forum has a number of representatives that sit on a variety of integrated care boards and sub-groups across the district:
