Practical Support for Bradford Integrated Community Teams (ICTs)
Voluntary & Community Sector (VCS) organisations are an integral part of Integrated Community Teams (ICTs).
ICTs in Bradford have been set up to support adults with long term and often complex conditions. Their key feature is that they pull together a team of professionals from different health, social care and voluntary and community organisations to provide joined up care packages.
To support VCS organisations work effectively with ICTs, the Bradford ICT Sub-Group (Bradford Integrated Care for Adults Programme) is offering a range of development tools and resources. These have been developed following feedback, comments and shared learning across all of the Bradford ICTs. Specifically, you can now access:
1. Refreshed Operational Blueprint: This brings together practical tools and resources to help you develop and implement an effective ICT.
2. Flexible Support Programme: This offers a range of support to address specific development needs, covering a range of important topics such as:-
a. Setting up and running effective ICT meetings
b. Establishing key processes and ways of working
c. Clarifying roles and responsibilities
d. Using IT systems to support care planning and records
e. How to identify cases uses a range of tools and approaches beyond the risk stratification tool.
f. Providing joined up care packages through multi-disciplinary working in partnership across health, social care, voluntary and community services - building robust networks
