Healthy You
Healthy You is an exciting new project designed to support people experiencing serious mental illness to become healthier, and more active. The project is being piloted in 3 areas across the District. The sessions can only be accessed through GP referral.
The programme is funded by Mind In Bradford and led by CNet, the sessions will finish in November 2022.
The 3 interventions are:
Healthy Eating & Weight Loss
Information & guidance on healthy food shopping, alternatives to takeaways, the health risks of certain foods, awareness of the link between food and mood, relaxation techniques to help with stress, boredom and anxiety.
Small Steps Exercise
Gentle exercise in short sessions, helping to set achievable goals to increase exercise, information on the effect of exercise on mood & physical health.
Smoking Reduction
Smoking Reduction Information on the effect of smoking on the body, how to be motivated to quit & methods of choosing new habits and routines to replace smoking.
The sessions are open to people who have a diagnosis of serious mental illness e.g. bipolar disorder, schizophrenia, psychosis and severe depression. Participants need to be referred to the project by a GP, social prescriber or other health professional in PCN 6, Modality and BD4+/PCN 9 areas. Participants must be registered at one of the following GP practices to be eligible to attend:

The referral form is short and easy to complete. It can be downloaded from the link below and the completed form can be uploaded onto the same secure portal. We will then contact the person being referred with details of the sessions they would like to attend. Participants can also bring along a person to support them in the session.
